We are seeking a highly skilled and compassionate registered nurse to join our specialist team as a Clinical Nurse Specialist. This is an exciting opportunity to work autonomously, managing a caseload of patients with complex symptom management needs in the community.
As a Clinical Nurse Specialist, you will provide palliative nursing interventions, working closely with medical and other colleagues to ensure a coordinated and effective service in the hospice and community setting.
Key Responsibilities- Assess referrals for appropriateness and urgency, and identify nursing best practice developments to support the clinical team.
- Work autonomously, managing a caseload of patients with specialist palliative care and end of life care needs in the community.
- Provide palliative nursing interventions, promoting skilled, professional, and compassionate nursing care within the community.
- Collaborate with medical and other colleagues to ensure a coordinated and effective service in the hospice and community setting.
We are looking for a self-motivated, kind, and caring registered nurse with excellent communication and assessment/prioritisation skills. The successful candidate will need to be able to work autonomously and demonstrate advanced nursing practice.
Own transport is essential for this role.
