Job Overview

We are seeking a highly motivated and experienced Care Coordinator to join our Ealing Community Rehabilitation Team. As a key member of our multidisciplinary team, you will play a vital role in providing innovative and recovery-focused care to individuals with complex mental health needs.

Main Responsibilities

The successful candidate will be responsible for:

  • Assessing, planning, implementing, and evaluating comprehensive care packages for individuals with severe and enduring mental health problems.
  • Developing and delivering psychosocial interventions to support individuals and their families.
  • Working collaboratively with multidisciplinary teams to provide holistic care and support.
  • Providing expert advice and guidance to colleagues and stakeholders.
  • Participating in audit and research activities to improve service delivery.

Requirements

To be successful in this role, you will need:

  • Experience of working in multidisciplinary mental health teams.
  • Excellent communication and interpersonal skills.
  • Able to work autonomously and as part of a team.
