Care Coordinator Mental Health Nursing/Social Worker/OT Specialist

Job Overview
We are seeking a highly motivated and innovative Care Coordinator to join our Ealing Community Rehabilitation Team. As a key member of our multi-disciplinary team, you will play a vital role in delivering individualized and recovery-focused care to individuals with complex care needs related to psychosis and co-existing substance use and physical health problems.
Main Responsibilities
As a Care Coordinator, you will be responsible for assessing, planning, implementing, and evaluating comprehensive care packages for our clients. You will work closely with our team to develop and deliver individualized programs of intervention and support, using a wide range of resources. Your excellent communication and interpersonal skills will enable you to build strong relationships with clients, carers, and other professionals.
Requirements
- Current professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy
- Registration with a professional body such as the NMC or other relevant body
- Evidence of post-registration training and CPD
- Experience of working in multi-disciplinary mental health teams with clients with severe and enduring mental health problems
- Knowledge of the CPA process and excellent verbal and written communication skills
