Care Coordinator Role at Mega Nursing & Care

We are seeking a dedicated and compassionate Care Coordinator to join our expanding team at Mega Nursing & Care. The successful candidate will play a crucial role in providing exceptional care and support to individuals within our community.

About the Role

The Care Coordinator will be responsible for conducting assessments with new service users and conducting regular reviews with existing clients. They will also prepare support plans, care plans, and all necessary documents for service users' folders, ensuring accuracy and compliance.

Main Responsibilities

  • Conduct client assessments and reviews
  • Prepare support plans and care plans
  • Build and maintain relationships with stakeholders
  • Initiate and manage care packages
  • Prepare rota every two weeks
  • Train and supervise senior support workers
  • Liaise with social workers
  • Handle issues and concerns
  • Report to the branch manager
  • Provide coverage for care calls and participate in on-call duties
  • Assist with induction and training of new staff
  • Conduct spot checks and supervisions
  • Participate in staff meetings
  • Represent the company in a professional manner
  • Carry out risk assessments
  • Assist with satisfaction surveys and monitor staff performance
  • Maintain confidentiality and adhere to data protection guidelines
  • Apply principles of the Mental Capacity Act
  • Cover care calls responsibilities
  • Report issues and concerns
  • Provide care with service users' consent

Requirements

  • At least 1 year experience as Care Coordinator (Domiciliary Care)
  • At least Level 3 in Health & Social Care
  • Full valid UK Driving Licence
  • Ability to use own car for work purposes

Working Times

Monday - Friday + Every Other Weekend (between 7am - 10pm)
