Medway Community Mental Health Team is seeking a dedicated Duty Worker to join our team in Gillingham. This Band 6 role offers an exciting opportunity to provide mental health assessments, advice, and support to individuals in need.

Key Responsibilities:
  1. Conduct comprehensive assessments of patients' mental health needs
  2. Develop and implement personalized care plans
  3. Collaborate with patients, families, and healthcare professionals to ensure seamless care
  4. Champion best practices and quality improvement initiatives
Requirements:
  1. Registered Mental Health Nurse with a strong understanding of CPA and care coordination
  2. One year experience at Band 5 or above in hospital or community healthcare
  3. Evidence of continued professional development, appropriate to role
  4. Clear understanding of the Recovery Approach and its application in practice
