We are seeking a passionate and experienced Adult Nurse Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that meet their unique needs.
Key Responsibilities:- Design and deliver tailored care plans that address the physical and physiological needs of clients
- Effectively manage the multidisciplinary team through strong communication and teamwork
- Provide ongoing monitoring, support, and assessment of clients' needs
- Deliver nursing interventions, including medication management
- Build and maintain relationships with clients and their families
- Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
- RGN with an active NMC PIN
- Supportive management style that brings out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills for effective liaison with patients, families, and the MDT
- Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs
- Up to £45,000 per annum DOE
- 1 day working from home per week/4 days in the community
- Monday to Friday (9-5) hours
- 33 days holiday

- Business travel expenses of £0.45 per mile
