About the Role
We are seeking a skilled Social Worker to join our Children with Disabilities Team. As a Social Worker, you will work with children and young people who have significant and profound disabilities, providing specialist support and guidance to help them achieve their full potential.
Key Responsibilities
- Assess the needs of children and young people with disabilities, identifying areas where specialist support is required.
- Develop and implement care plans to meet the needs of children and young people, working closely with families, health professionals, and other agencies.
- Provide direct support to children and young people, using a range of methods to communicate and engage with them.
- Work collaboratively with other professionals, including health and education teams, to ensure a coordinated approach to supporting children and young people.
- Contribute to the development of the Children with Disabilities Team, sharing knowledge and expertise to improve services and outcomes for children and young people.
About You
We are looking for a Social Worker who is passionate about working with children and young people with disabilities. You will have a strong understanding of the needs of this group and be able to provide specialist support and guidance. You will also have excellent communication and interpersonal skills, with the ability to work effectively with families, professionals, and other stakeholders.
