About the Role

We are seeking an experienced and motivated Practice Nurse to join our team at County Surgery. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Key Responsibilities
  • Develop and implement health promotion and well-being programs to improve patient outcomes.
  • Undertake child vaccination programs and provide routine nursing care to patients as required.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide guidance to patients on the use of prescribed and over-the-counter medications, including side effects and dosages.
  • Oversee the provision of wound care services to patients and request pathology services as necessary.
  • Maintain accurate clinical records in accordance with legislation and ensure read codes are used effectively by the nursing team.
  • Develop, implement, and participate in well-woman and well-man clinics, as well as chaperone patients when necessary.
  • Assist GPs with minor surgery and prioritize health issues, intervening appropriately to ensure patient well-being.
  • Support the team in dealing with clinical emergencies and recognize, assess, and refer patients presenting with mental health needs.
  • Liaise with external services and agencies to ensure patients receive appropriate support, and delegate clinical responsibilities safely and effectively.
  • Support the clinical team with safeguarding matters, substance abuse, and addictive behavior, referring patients as necessary.
  • Deliver opportunistic health promotion and manage chronic disease as required, making home visits and conducting clinical appointments and reviews as needed.
  • Develop and review audit protocols and processes, implementing nursing protocols in conjunction with partners, and supporting the nursing team with guidance, appraisals, and training needs analyses.
  • Participate in local initiatives to enhance service delivery and patient care, supporting shared learning within the practice and continually reviewing clinical practices in response to national policies and initiatives.
  • Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs, and lead the planning and implementation of changes within the nursing team to enhance patient care.
  • Support the practice team with reviewing and implementing practice policies and protocols, ensuring conformance to legislation, and act as a clinical leader in the delivery of practice nursing services, prioritizing patient needs.
  • Lead the nursing team in planning and implementing local guidelines, protocols, and standards, and participate in practice team meetings, delivering the nursing agenda and running nurse department meetings.
  • Promote a learning environment with a no-blame culture, leading the QoF strategy and delivery for LTC management, maximizing income, and managing and maximizing CCG, PHE, and NHSE-driven targets.
  • Support the development of others to maximize staff potential, leading others to make realistic self-assessment of their knowledge and skills, and taking responsibility for own development, learning, and performance.

  • Postgraduate diploma or degree in Chronic Disease Management (desirable)
