Key Responsibilities
The successful candidate will collaborate with a specialized MDT to support the implementation and execution of a thorough cardiac and heart failure rehabilitation program. Responsibilities will include:
- Conducting clinical observations

Job Duties
The role encompasses a variety of responsibilities, including:
- Completing an approved orientation program
- Supporting a patient-centered clinical environment
- Assisting attendees of the Heart Failure Cardiac Rehabilitation Program under the guidance of the Clinical Nurse Specialist (CNS)
- Gathering initial information for screening by the CNS
- Providing clinical care, recording observations, and performing assigned clinical duties
- Conducting venepuncture and 12-lead ECGs
- Assisting the CNS in managing unwell patients and participating in resuscitation efforts
- Maintaining patient privacy and dignity while adhering to health board policies
- Utilizing health improvement tools to engage patients in discussions about their well-being
- Delivering educational information to program attendees in group or individual settings
- Providing accurate verbal reports to the CNS and MDT members
- Communicating sensitively with patients and their families while ensuring confidentiality
- Recognizing personal limitations and seeking guidance from the CNS when necessary
- Participating in regular clinical supervision
- Documenting all clinical activities accurately
- Engaging with diverse service users and their families
Technical and Administrative Responsibilities
Additional duties include:
- Managing filing and stock control
- Participating in data collection for audits
- Contributing to general housekeeping within the department
- Ensuring the safety and cleanliness of assessment and treatment equipment
- Providing necessary forms and literature for treatment sessions
Professional Development
To ensure continuous improvement, the candidate will:
