Key Responsibilities- Assess and plan care for children with disabilities, working in partnership with families and other healthcare professionals.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each child and family.
- Provide expert clinical advice and guidance to other healthcare professionals, ensuring that children receive the best possible care.
- Work collaboratively with other agencies and services to ensure seamless care coordination and support for children and families.
- Contribute to the development and implementation of policies and procedures that promote high-quality care and improve outcomes for children with disabilities.
- Registered Nurse with a valid NMC registration.
- Post-registration study or qualification in a relevant area, such as pediatric care or community health.
- Significant post-registration clinical experience, preferably in a community setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with children, families, and other healthcare professionals.
- Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the changing needs of children and families.
