Community Health Nurse for Children with Disabilities

We are seeking a skilled and compassionate District Nurse to join our Community Team in East and West Sussex. As a key member of our team, you will work closely with individuals and their carers to develop personalized care plans and coordinate care across the health and social care system.
About the Role- Provide advanced assessment and clinical care to individuals with complex needs, promoting health and preventing unnecessary hospital admissions.
- Interpret clinical signs and symptoms, develop treatment plans, and provide advanced clinical advice to the healthcare team.
- Develop and analyze audit data to enhance clinical practice and improve patient outcomes.

- Post-registration study or qualification
- Mentorship or equivalent/practice assessor
- Leadership/management course or willingness to work towards
- District Nurse qualification
- Significant post-registration clinical experience
- Significant experience of working within a community setting
