About the Role

Southern Health NHS Foundation Trust is seeking a skilled Health Care Assistant to join our dedicated team of Nurses within the Alton Community Nursing Team. As a Health Care Assistant, you will play a pivotal role in delivering high-quality patient care and contributing to the prevention of hospital admissions and re-admissions.

Main Responsibilities

  • Develop and implement collaborative care plans, working closely with patients, families, and healthcare professionals to ensure comprehensive and person-centered care.
  • Provide essential educational and advisory services to patients, empowering them to manage their health and well-being effectively.
  • Conduct wound management, catheter care, and medicines management interventions to promote optimal patient outcomes.
  • Maintain effective communication with patients, relatives, and healthcare teams, utilizing a range of communication skills to address complex and sensitive issues.
  • Document patient care and progress accurately and efficiently, adhering to organizational policies and procedures.

Person Specification

  • Foundation Degree in Health & Social Care or equivalent experience.
  • Full Driving License.
  • GCSE grade A-C or equivalent in English and Maths (desirable).
