Job Summary

We are seeking an experienced and motivated Specialist Community Nurse to join our team at Wiltshire Health and Care LLP. As a key member of our community team, you will be responsible for delivering high-quality care to patients in their homes and residential settings.

About the Role

The successful candidate will have a strong background in community nursing, with experience in case management, leadership, and teaching. You will be expected to work collaboratively with a multidisciplinary team, including nurses, therapists, and other healthcare professionals, to provide comprehensive care to patients.

Key Responsibilities
  • Deliver high-quality care to patients in their homes and residential settings
  • Work collaboratively with a multidisciplinary team to provide comprehensive care
  • Develop and implement care plans that meet the individual needs of patients
  • Provide education and support to patients and their families
  • Contribute to the development of community-based services and initiatives
Requirements
  • Registered nurse with a strong background in community nursing
  • Postgraduate community qualification or equivalent experience
  • Leadership experience and ability to work collaboratively with a team
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
