Job Title: Supported Living Worker

About the Role

We are seeking a dedicated and compassionate Supported Living Worker to join our team at Home Group. As a Supported Living Worker, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community.

Key Responsibilities

  • Assist customers with practical daily living tasks to promote independence and achieve their goals.
  • Support customers with daily living activities such as meal preparation, shopping, budgeting, and maintaining their tenancy.
  • Accompany customers to appointments, engage in hobbies and interests, and support them in attending college or work.
  • Provide personal care and medication support as outlined in the customer's support plan.
  • Conduct risk assessments, support planning, goal setting, and regular reviews to ensure the customer's needs are met.

Requirements

  • A Level 2 Health and Social Care Certificate or willingness to work towards it.
  • Experience of caring for others, either in a professional or personal capacity.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach.
  • Enhanced DBS check and ability to use technology to update support plans and collaborate with colleagues.
