About the Role
We are seeking a highly motivated and compassionate Supported Living Specialist to join our team at Home Group. As a Supported Living Specialist, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community.
Key Responsibilities
- Assist customers with practical, daily living tasks to develop their independence and help them achieve their goals, aspirations, and outcomes documented in their support plans.
- Support customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy, and moving onto independent accommodation if they are able.
- Accompany customers to appointments, help them participate in hobbies and interests, or attend college or work.
- Support customers with their personal care and medication needs outlined in their support plan.
- Conduct risk assessments, support planning, goal setting, and regular reviews.
Requirements
- A Level 2 Health and Social Care Certificate or willingness to work towards it.
- Passion for supporting customers to live their best life, working collaboratively with an eye for detail.
- Experience of caring for others, either in a care setting or through caring for family members or children.
- Ability to work independently, remain calm under pressure, and demonstrate a resilient approach.
- A flexible, person-centered approach, listening to customers to understand their needs and recognizing that every customer is unique.
