Key Responsibilities- Caseload Management: You will work with a caseload of clients, providing individualized support and guidance to help them achieve their goals.
- Needs Assessment: You will conduct thorough needs assessments to identify clients' strengths, challenges, and goals, and develop personalized support plans to address these.
- Community Linkage: You will build relationships with outreach workers, other agencies, and community partners to ensure seamless support and services for clients.
- Advocacy and Support: You will advocate on behalf of clients to access benefits, services, and resources, and provide emotional support and guidance to help them navigate complex systems.
- Skills Development: You will work with clients to identify and build on their strengths, talents, and skills, and support them in developing practical skills such as budgeting, cooking, and tenancy management.
- Health and Wellbeing: You will support clients to access healthcare services, manage mental and physical health, and develop healthy habits and coping strategies.
- Record Keeping: You will maintain accurate and up-to-date records of client interactions, progress, and outcomes, using relevant systems and tools.
