The REACT team operates as a cohesive, multi-disciplinary community service within the healthcare framework, encompassing Urgent Community Response services, Outpatient Parenteral Antibiotics Therapy (OPAT), Out of Hours services, and Virtual Ward pathways.
This position is focused on our Urgent Community Response Team, which has established collaborative pathways with emergency medical services and innovative virtual care initiatives.
Our team is committed to delivering outstanding, patient-centered care to individuals within the community.
Key Responsibilities:
- Deliver expert nursing care with specialized knowledge in preventing hospital admissions, managing frailty, and supporting elderly patients in the community.
- Provide professional guidance and leadership, including recruitment, supervision, and development of nursing staff.
- Exhibit strong leadership during shifts to ensure optimal patient flow and staffing efficiency.
- Promote high standards of clinical practice and lead the delivery of quality healthcare.
- Design and implement specialized training programs for nursing and allied health professionals focused on admission prevention.
- Maintain an up-to-date understanding of relevant research and national guidelines regarding elder care and admission avoidance.
- Collaborate with service leads and financial advisors to ensure effective budget management and departmental performance.
- Be accountable for ensuring compliance with clinical governance and quality standards within the REACT team.
