About the Role

Oxfordshire Mind is seeking a skilled Community Connector to join our team. As a Community Connector, you will play a vital role in supporting patients and championing them to access a wide range of community services and resources.

Key Responsibilities
  • Actively support patients to access community services and resources, including social care, housing, family, debt, and employment counseling.
  • Work in partnership with Berkshire Healthcare Foundation Trust to test new and integrated models of primary and community mental health care.
  • Support patients to identify their own goals and help them pursue these to maintain good health and wellbeing.
  • Collaborate with colleagues to provide a comprehensive service to patients.
  • Travel to different sites daily to deliver face-to-face sessions across the Berkshire region.
Requirements
  • Understanding of good practice in safeguarding Adults.
  • Experience of working within mental health, wellbeing, or similar environments.
  • Ability to work as part of a team and communicate effectively.
  • Excellent listening and interpersonal skills.
  • Ability to confidently use IT systems and software at a basic level.
