About the Role

We are seeking a highly motivated and caring Community Support Worker to join our Home Treatment Team in Newbury. As a key member of our team, you will work alongside Mental Health Practitioners to provide intensive care and treatment in the home of individuals with functional mental health problems and dementia.

The successful candidate will have a passion for working with older people and will be able to provide high-quality mental health interventions. You will be well-supported by the Team Lead and Mental Health Practitioners within the Home Treatment Team.

Key Responsibilities
  • Provide social and emotional support to patients and carers in a trusting and therapeutic relationship.
  • Enable patients to maintain and develop their social skills, encourage social contacts, and promote social networks as appropriate.
  • Assist in the assessment and meeting of care needs of clients and their carers in the community.
  • Participate in individual therapies in patients' homes and in the community, contributing to the jointly agreed care plan under the supervision of qualified staff.
  • Co-facilitate and support staff in group activities.
  • Support Mental Health Practitioners in providing psychosocial assessment and intervention for behaviours that challenge in dementia.
  • Maintain clinical records as requested by a qualified member of staff.
  • Participate in clinical supervision and appraisal.
