About the Role

We are seeking a highly skilled and experienced Field Care Supervisor to join our team at City & County Healthcare Group. As a Field Care Supervisor, you will play a critical role in supporting the delivery of high-quality care services to our clients.

Key Responsibilities
  • Support the planning and coordination of care services to ensure seamless delivery of holistic packages of individual care support.
  • Conduct regular reviews of individual services, including repeated and renewed needs and risk assessments.
  • Participate in multi-disciplinary reviews of service users' needs and develop care plans that meet their unique requirements.
  • Provide on-site supervision and evaluation of care worker practice to ensure competence and adherence to company standards.
  • Collaborate with Care Coordinators to develop and implement effective care worker rotas that meet the needs of clients and care workers.
Requirements
  • Experience in carrying out initial risk and needs assessments in respect of individual service users.
  • Ability to work in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure seamless delivery of care services.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, care workers, and other stakeholders.
  • Ability to work independently and as part of a team, with a strong focus on delivering high-quality care services.
