Gray Healthcare is seeking an experienced Male Support Worker to join their team in Telford, supporting a gentleman with a mild learning disability, Schizo-Affective disorder, and Epilepsy.
Key Responsibilities
- Provide high-quality support to individuals with complex needs, including those with learning disabilities and mental health conditions.
- Develop and implement personalized care plans, taking into account the individual's needs, preferences, and goals.
- Work collaboratively with a multidisciplinary team, including clinicians, occupational therapists, and Positive Behavioural Support (PBS) specialists, to ensure comprehensive support.
- Contribute to a safe and supportive environment, promoting the individual's independence, dignity, and well-being.
- Participate in ongoing training and professional development to enhance skills and knowledge in supporting individuals with complex needs.
Requirements
- Experience in providing support to individuals with complex mental health needs and learning disabilities.
- Ability to understand and implement detailed care plans and risk assessments.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
- Flexibility and adaptability, with a willingness to work varied shifts, including evenings and weekends.
