Key Responsibilities- Develop and implement individualized care plans to address the unique needs of each family and youth.
- Build and maintain strong relationships with families, youth, and community partners to ensure seamless care coordination.
- Provide education and support to families on mental health topics, including crisis intervention and trauma-informed care.
- Collaborate with multidisciplinary teams to ensure comprehensive care and services are provided.
- Bachelor's degree in social work, psychology, education, or a related field.
- Two years of experience working with youth and families in a mental health or human services setting.
- Ability to use own vehicle for local travel, if necessary.
- Competitive hourly rate of $84/hour, with regular wage increases and bonuses.
