Position Overview

The Northern Care Alliance NHS Foundation Trust is seeking a motivated and proactive Rehabilitation Support Specialist to join our dedicated Home in a Day Team. This team is committed to delivering patient-centered evaluations and therapeutic interventions within a hospital environment, with the goal of facilitating a smooth transition for patients back to their residences.

Key Responsibilities

As a Rehabilitation Support Specialist, you will collaborate closely with Occupational Therapists and Physiotherapists to conduct assessments and implement treatment plans aimed at helping patients regain their autonomy and ensure a safe discharge process.

A valid UK driving license and access to a personal vehicle for work-related duties are essential, as you will be required to operate the Home in a Day vehicle to assist with patient discharges.

You should possess flexibility, strong communication abilities, and a passion for teamwork. Our team is dedicated to providing high-quality service to our patients, emphasizing collaborative efforts. Self-motivation and the ability to work independently as well as part of a team are crucial. Essential Skills & Qualifications

  • Proficient communication skills (both written and verbal)

  • Understanding of OT/physiotherapy assessments and interventions
  • Conflict resolution skills

Required Qualifications

  • NVQ Level 3 in Healthcare or a Level 3 Senior Healthcare Support Worker Apprenticeship, or equivalent experience
  • Level 1 Literacy and Numeracy (GCSE English and Maths Grade D-G)
