Job Summary

We are seeking a highly motivated and compassionate Field Care Supervisor to join our team. As a Field Care Supervisor, you will be responsible for delivering outstanding care and support to our clients, while also providing leadership and guidance to our care team.

Key Responsibilities

  • Conduct thorough risk assessments and develop person-centered care plans for each new client
  • Complete quality monitoring visits with clients to ensure high standards of care are maintained
  • Review and update care plans regularly to reflect changing client needs
  • Monitor and manage medication administration, including electronic and paper-based MAR charts
  • Provide on-call support to ensure seamless service delivery during out-of-hours periods
  • Liaise with healthcare professionals, including social workers, occupational therapists, and GPs, to ensure comprehensive care coordination
  • Coach and mentor new care staff to ensure they have the skills and confidence to deliver high-quality care

Requirements

  • Be a caring and compassionate individual with a strong commitment to delivering outstanding care
  • Have the ability to work effectively in a pressured environment, prioritizing tasks and managing multiple responsibilities
  • Hold a valid UK driving license and have access to a reliable vehicle
  • Have a strong understanding of care principles and practices, with a willingness to complete NVQ level 3 training
