About the Role

We are seeking a highly motivated and compassionate Support Practitioner to join our team in West Dunbartonshire. As a Support Practitioner, you will play a vital role in supporting individuals with their everyday lives, enabling them to achieve their own ambitions and goals.

**Key Responsibilities:**

  • Support and monitor individuals with their healthcare needs, including administering medication and personal care
  • Promote independence and individuality according to individuals' My Support Plans
  • Record daily tasks and maintain accurate up-to-date records
  • Work as part of a team, effectively communicating with staff, families, and individuals supported

  • Strong work ethic and organized to support routines
