Position Overview
The Community Support Specialist is responsible for conducting comprehensive assessments under the Care Act, utilizing both in-person and telephonic methods based on the assessed risk levels and individual needs. This role emphasizes the importance of mental capacity considerations throughout the assessment process.
In collaboration with a network of supportive colleagues, citizens, caregivers, and both statutory and community partners, the Specialist will facilitate the delivery of tailored support services. The aim is to foster independence, choice, and control for individuals, while also promoting early intervention and community-led support initiatives that enhance overall well-being and address local needs.
Key Responsibilities
- Conduct assessments focusing on adult needs within a person-centered framework.
- Engage with adults facing various challenges, including learning disabilities, mental health issues, substance misuse, and physical health concerns.
- Implement risk management strategies while demonstrating flexibility and resilience in adapting to evolving situations.
Candidate Profile
- Proven experience in social care, health, or voluntary sector environments.
