Position Overview

We are pleased to announce an opportunity to become a vital member of our forward-thinking frailty team. As a dedicated and skilled Band 8A Advanced Clinical Practitioner, you will be committed to enhancing service delivery and ensuring exceptional patient-focused care.

Your role will focus on preventing hospital admissions for frail elderly individuals within the community, facilitating their safe return home, and contributing to the establishment of a same-day assessment unit for frailty cases.

The successful candidate will engage as a proactive and innovative ACP within our thriving Integrated Frailty service, which operates seven days a week, providing comprehensive assessments for the elderly population.

You will collaborate with a supportive and professional team that includes Geriatricians, Nurses, Occupational Therapists, Physiotherapists, Coordinators, Advanced Clinical Practitioners, Pharmacists, Social Services, and Community Services.

Key Responsibilities

The selected candidate will function as an independent practitioner, actively participating in the stabilization and management of acute care pathways through advanced assessments and treatment of frail elderly patients. You will also play a crucial role in admission avoidance and urgent care responses, while supporting the development of front door frailty services.

Job Duties

PRIMARY OBJECTIVES

  • Provide clinical expertise and leadership to the integrated clinical team, utilizing advanced assessment, diagnosis, and treatment skills, including independent prescribing.
  • Exhibit critical thinking in clinical decision-making, collaborating with the Care of the Elderly Virtual Ward multidisciplinary team to address patient needs while adhering to policies and procedures.
  • Mentor and support trainee nurse practitioners, offering consultancy and advisory services to the multidisciplinary team, leveraging your expert knowledge.
  • Deliver educational services to students, medical, and nursing staff.
  • Facilitate the development of an integrated care pathway, coordinating with social care and primary care colleagues to promote admission avoidance.
  • Oversee operational management, performance monitoring, professional leadership, and service enhancement of the virtual ward.

Qualifications and Experience

Essential Qualifications

  • Significant post-registration experience, including leadership roles at Band 7 or above.
  • Experience in teaching and mentoring across the multidisciplinary team.
  • Non-medical prescribing certification.

Desirable Qualifications

  • Experience in nurse-led triage.
  • Development of clinical protocols and guidelines.
  • Team leadership experience.
  • Involvement in audits and research.
  • Project management experience.
  • Background in frailty or acute community care.
  • Experience in palliative and end-of-life care.

Skills Required

Required Qualifications

  • Master's degree in Advanced Clinical Practice.
  • Certification as an independent prescriber.
