Job Title: Young Person's Practitioner

Job Summary:

Catch22 is seeking a dedicated and compassionate Young Person's Practitioner to join our team. As a key member of our Risk and Resilience Service, you will play a vital role in supporting children and young people who are at risk of exploitation, substance misuse, or going missing from home.

Main Responsibilities:

  • Provide tailored one-to-one support, guidance, and interventions to children and young people.
  • Assess the needs of children and young people, determine appropriate support plans, and inform risk reduction plans.
  • Ensure high-quality referrals, support, care, and risk management, with ease of transition.
  • Develop sustainable joint service work with referring agencies, local authorities, and children's social care.
  • Ensure timely and accurate data recording, including tracking, outcome monitoring, and KPI monitoring.
  • Provide consultation, information, training, and presentations to stakeholders from various settings.

Requirements:

  • 5 GCSEs (grade C or above) or equivalent.
  • Experience working with children and young people with complex needs or vulnerabilities.
  • Preferable: Working knowledge of supporting children and young people in areas such as substance misuse, child exploitation, or missing from home.
