Job Overview

We are seeking a skilled and compassionate Clinical Support Worker to join our team in providing high-quality care to older adults living with dementia and other mental health conditions in their own homes and care settings.

Main Responsibilities
  • Provide person-centered care to individuals with dementia and other mental health conditions, promoting independence and well-being.
  • Work collaboratively with multidisciplinary teams to develop and implement effective care plans.
  • Conduct assessments to identify needs and develop strategies to reduce hospital admissions and improve quality of life.
  • Provide education and support to care home staff to enhance their knowledge and skills in caring for residents with dementia.
  • Work in a fast-paced environment, responding to urgent situations and prioritizing tasks effectively.
Requirements
  • Minimum 5 GCSEs A-C, including English and Maths.
  • Clinical experience working with adults with mental health problems in a health or social care setting.
  • Ability to form respectful relationships with diverse individuals and teams.
  • Basic knowledge of systemic understandings of mental health difficulties and the impact of dementia on older adults.
  • Ability to work in a team, engage in discussions, and solve interpersonal issues.
  • Basic knowledge of Microsoft Office and ability to use it effectively.
  • Valid driver's license and access to a car for travel to care homes and service users' homes.
