Job Overview

The Nurse Practitioner will operate within their professional scope to deliver comprehensive patient care, encompassing initial assessment, clinical evaluation, diagnosis, treatment, and ongoing care management both in the clinic and during home visits. This role requires a high level of critical thinking and clinical judgment.

The successful candidate will enhance our current team of dedicated Practice Nurses and Healthcare Assistants, who manage essential nursing tasks, health promotion, chronic disease management, and minor illness evaluations. You will be expected to work independently, conducting face-to-face clinical assessments during scheduled appointments, making informed clinical decisions based on your expertise and analytical skills.

The applicant must possess a Nurse Prescriber qualification and be prepared to conduct home visits, make necessary clinical referrals, and manage pathology results and hospital communications for patients.

Main Responsibilities:

  • Assist with daily operational duties
  • Prescribe and provide guidance on medications
  • Manage minor injuries and illnesses
  • Conduct triage and history taking
  • Work autonomously while maintaining composure under pressure
  • Exhibit leadership qualities
  • Participate in Practice Meetings
  • Conduct audits of nursing practices
  • Refer patients to appropriate healthcare professionals within the Primary Care Team and external agencies, involving GPs as necessary
  • Collaborate as part of a coordinated nursing team and engage in joint initiatives
  • Attend training sessions to uphold professional qualifications and expertise
  • Demonstrate a commitment to continuous professional development

Qualifications:

  • Registered General Nurse
  • Nurse Practitioner Degree
  • Non-Medical Independent Prescriber

Experience:

  • Several years of post-registration experience
  • Experience in managing chronic conditions
  • Experience in minor illness management, clinical decision-making, and telephone triage
  • Proven ability to work independently

Skills:

  • In-depth knowledge of general practice operations
  • Current understanding of health agendas and disease management
  • Excellent interpersonal and communication skills
  • Ability to collaborate effectively within a multidisciplinary team
  • Proficient in clinical examination and diagnostic techniques
  • Familiarity with prescribing guidelines and legal/ethical considerations
