Job Title: Advanced Nurse Practitioner

Chapelford Medical Centre is seeking an experienced and skilled Advanced Nurse Practitioner to join our team. As a key member of our primary care team, you will play a vital role in providing high-quality patient care and contributing to the development of our practice.

Job Summary

The Advanced Nurse Practitioner will be responsible for providing comprehensive and coordinated care to patients with complex health needs. This will involve conducting thorough assessments, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care transitions.

Key Responsibilities
  • Conduct thorough assessments of patients with complex health needs, including history taking, physical examination, and diagnostic testing.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and their families to promote health literacy and self-management.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Contribute to the development and implementation of practice policies and procedures.
Requirements
  • Registered General Nurse with a Nurse Practitioner degree and non-medical independent prescriber qualification.
  • Several years of post-registration experience, with a strong background in managing long-term conditions and minor illnesses.
  • Experience in clinical decision-making, telephone triage, and working autonomously.
  • Knowledge of the working of general practice, including health agendas, local and national management of diseases, and undiagnosed health roles.
  • Excellent interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team.
