Job Summary

The Chapelford Medical Centre is seeking an experienced and skilled Advanced Nurse Practitioner to join our team. As a key member of our multidisciplinary practice team, you will be responsible for providing high-quality patient care, working autonomously, and making clinical decisions based on your expertise and knowledge.

Main Responsibilities
  • Supporting on-the-day duties, including triage, prescribing, and advising on medication
  • Managing minor injuries and illnesses, and working effectively in a fast-paced environment
  • Leading and mentoring junior staff members, and contributing to the development of our nursing team
  • Attending practice meetings, performing audits, and referring patients to appropriate disciplines within the Primary Care Team
  • Working collaboratively with GPs, other healthcare professionals, and external agencies to ensure seamless patient care
  • Staying up-to-date with the latest clinical guidelines, research, and best practices in primary care
Requirements
  • Registered General Nurse with a Nurse Practitioner Degree and Non-Medical Independent Prescriber qualification
  • Several years of post-registration experience, with a strong background in managing long-term conditions, minor illnesses, and clinical decision-making
  • Excellent interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team
  • Knowledge of the working of general practice, including health agendas, local and national management of diseases, and undiagnosed health roles
  • Ability to work autonomously, with a strong sense of leadership and initiative
