Key Responsibilities:- Assess and develop individualized plans to support children and young adults with disabilities
- Collaborate with multidisciplinary teams to provide holistic care and support
- Work closely with families and carers to ensure their needs are met and their voices are heard
- Develop and maintain effective relationships with service users, families, and professionals
- Stay up-to-date with the latest research, policies, and best practices in children's social care
- Diploma or degree in Social Work, CQSW, CSS, or equivalent qualification
- Registration with Social Work England as a registered Social Worker
- Demonstrable capability to practice in line with current professional standards
- Proficiency in case recording and information technology systems
- Strong analytical and problem-solving skills, with the ability to work independently and as part of a team
