Job Summary

Key Responsibilities
  • Assess, plan, implement, and evaluate care plans for children and young people with complex healthcare needs
  • Collaborate with professionals across agencies and sectors to ensure safe and effective service delivery
  • Provide training and support to staff working in packages to ensure safe care delivery
  • Manage a clinical caseload in the community setting, completing assessments and bespoke care delivery plans
  • Clinical Governance and Risk Management
  • Work as part of the Education Team in providing classroom-based training to community staff
  • Educate, support, and complete competency assessments for non-registered healthcare staff on clinical delegated tasks
  • Carry out staff appraisals and supervisions for registered nurses and Healthcare Assistants
Requirements
  • UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills
  • Experience in managing complex healthcare needs and providing high-quality care and support
  • Ability to work collaboratively with professionals across agencies and sectors
  • Strong clinical governance and risk management skills
  • Ability to provide training and support to staff
