Job Overview

We are seeking a dedicated professional to provide leadership within the Intermediate Care team. This role is essential in ensuring that our services meet the highest standards while supporting the local community.

Key Responsibilities

  • Lead the Intermediate Care team, fostering a collaborative environment.
  • Engage with the locality's management team to enhance the Health & Wellbeing services.
  • Contribute to the strategic planning and development of services in partnership with key stakeholders.
  • Serve as a clinical leader, offering expertise and guidance to the integrated Locality team.
  • Collaborate with primary care networks to ensure safe and effective service delivery.
  • Build and maintain professional relationships with care homes providing Intermediate Care services.

Main Duties

  • Support individuals in expressing their needs and preferences.
  • Embrace diversity and respect the individuality of all clients.
  • Provide compassionate care by actively listening and engaging with clients.
  • Deliver nursing care and assessments in clients' homes, respecting their wishes.
  • Commit to continuous improvement by responding to feedback and implementing changes.
  • Work collaboratively to achieve optimal outcomes for clients.
  • Encourage teamwork and support colleagues in achieving shared objectives.
  • Act as a role model, valuing the contributions of all team members.

  • Preventing unnecessary hospital admissions.
  • Facilitating timely discharges from hospitals.
  • Avoiding long-term care placements whenever possible.

Qualifications and Skills Required

Essential

  • Degree or Graduate Diploma in health or social care.
  • Registration with HCPC, NMC, or SW(England).
  • Demonstrated commitment to professional development.
  • Leadership experience in a health or social care environment.

Desirable

  • Postgraduate qualification in leadership or project management.
