About the Role:
We are seeking a highly skilled and compassionate Care Coordinator to join our team at Peninsula Care Homes Ltd. As a Care Coordinator, you will play a vital role in ensuring the well-being and quality of life of our residents.
Key Responsibilities:
- Develop and implement care plans that meet the individual needs of our residents
- Provide emotional support and companionship to residents, promoting a sense of community and belonging
- Organize and lead activities that promote physical and mental well-being, such as exercise classes, arts and crafts, and social events
- Collaborate with other care staff to ensure seamless delivery of care services
- Monitor and report any changes in resident's condition to the care team
Requirements:
- Previous experience in a care setting, preferably in a similar role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility and adaptability to meet the changing needs of residents
- Basic knowledge of care principles and practices
