Position Overview

The role involves providing strategic leadership to the Intermediate Care team within the Torbay locality.

As a key member of the management team, you will offer guidance to the broader Health & Wellbeing Team, ensuring a cohesive approach to community health services.

Your contributions will be vital in shaping the strategic direction of the Torbay Locality, collaborating closely with the Deputy Community Service Manager, Community Service Manager, and other Professional Leads.

Additionally, you will play a crucial role in the development of Intermediate Care services, working in partnership with Community Service Managers and other Locality Leads.

Key Responsibilities

  • Provide inspirational clinical leadership within the integrated Locality team, ensuring high standards of service delivery.
  • Collaborate with primary care networks to guarantee safe and effective delivery of Intermediate Care services, aiming to reduce hospital admissions and facilitate timely discharges.
  • Establish and maintain professional relationships with Care Homes that provide Intermediate Care bed-based services.

About the Organization

The Intermediate Care team is dedicated to supporting individuals in Torbay who are acutely unwell, enabling them to manage their health safely and independently at home. Our primary objectives include:

  • Preventing unnecessary hospital admissions.
  • Facilitating timely discharges from hospitals.
  • Avoiding long-term care placements whenever possible.

We take pride in our person-centered approach, delivering high-quality, short-term interventions tailored to individual needs.

Additional Information

This position may have the potential to transition into a permanent role.

Qualifications and Skills Required

Essential:

  • Degree or Graduate Diploma in health or social care.
  • Current registration with HCPC, NMC, or SW(England).
  • Evidence of ongoing professional development.
  • Leadership experience in a health or social care environment.

Desirable:

  • Postgraduate qualification in leadership or project management.

Knowledge and Experience:

Essential:

  • Strong understanding of the goals and priorities of Intermediate Care.
  • Significant clinical expertise in your professional area.
  • Experience in managing complex cases.
  • Extensive background as a senior clinician or practitioner.
  • Proven ability in staff and resource management.
  • Experience in multidisciplinary teamwork across various agencies.
