About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our Home Treatment Team in Barrow. As a key member of our team, you will provide comprehensive assessments and triage services to individuals in acute mental health distress, supporting them to avoid hospital admission and facilitating early discharge from hospital.

Main Responsibilities
  • Provide evidence-based mental health assessments and signposting to alternative service provision for individuals requiring specialist urgent mental health assessment.
  • Develop and implement comprehensive, collaborative triage systems to ensure seamless onward facilitation of service users to the most appropriate care.
  • Work closely with GPs, Police, and Acute Hospital colleagues, as well as self-referrals and carer referrals, to provide high-quality care and support.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with a strong understanding of mental health care pathways and relevant interventions.
  • Excellent communication and customer service skills, with the ability to work sensitively and non-judgmentally with individuals in crisis.
  • Motivational and assertiveness skills, with the ability to work effectively in a team environment.
