Community Care Support Advisor

Position Overview: As a Community Care Support Advisor within the Health and Social Care Connect team, you will play a crucial role in guiding individuals through the various health and social care services available to them.
No prior experience in health or social care is required as we offer extensive training and a nurturing environment to help you understand the diverse range of support services accessible to adults in the community. Your role will not involve direct care or support work, but rather, you will provide a vital service to the community and collaborate with health and social care professionals, as well as partner organizations.
Work Environment: This position is office-based, with staff working on a shift rota covering hours from 8 PM to 8 AM, including weekends and public holidays. Additional pay incentives are available for night and weekend shifts. As this is a 24/7 service, there may be occasions where you are required to work day shifts, with initial training conducted during standard office hours.
Candidate Profile: We are seeking individuals who:
- Possess strong IT and communication skills;
- Have some experience in customer service, even if not specifically in health or social care;
- Are self-driven and resilient, capable of managing a high volume of referrals;
- Thrive in a dynamic and fast-paced environment.
