Job Summary

We are seeking a highly motivated and compassionate Mental Health Support Worker to join our team in the Primary Care Mental Health Service (PCMHS). As a key member of our team, you will work closely with adults who experience various moderate/complex mental health difficulties, providing psychological interventions and support to meet their biopsychosocial needs.

Main Responsibilities
  • Work alongside Primary Care Mental Health Practitioners to deliver a plan of care that meets the individual's needs.
  • Deliver psychological interventions, including confidence building, graded exposure, anxiety management, mood management, and guided self-help.
  • Support patients to attend other appointments that may be impacting their mental health presentation.
  • Integrate with GP practices, work alongside other agencies, and participate in multidisciplinary team discussions.
  • Manage and prioritize a caseload.
  • Support the team in completing physical health checks for patients registered on the Serious Mental Illness (SMI) register.
  • Contribute to service improvement and delivery initiatives.

The Primary Care Mental Health Service (PCMHS) is an integral part of Primary Care and community working, delivering care and treatment to adults presenting with moderate/complex mental health difficulties. This role provides a fantastic opportunity to work across all services, sharing a common goal to support the people of Sunderland, Washington, and Coalfields to improve their mental and physical health goals.

Person Specification

We are looking for individuals with the following qualifications, skills, and experience:

  • GCSE maths and English at C or above/equivalent adult learning qualification.
  • Level 3 qualification (A-level, NVQ3) in Care or Level 2 qualification (GCSE, NVQ2) in care, with a commitment to undertaking the relevant level 3 qualification on appointment to post.
  • Experience of working with service users with complex health needs.
  • Desirable qualifications include completion of training in psychological intervention and/or counselling skills.
