About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Mental Health Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Work collaboratively with Primary Care Mental Health Practitioners to deliver a comprehensive plan of care that meets the individual's biopsychosocial needs.
  • Deliver psychological interventions, including confidence building, graded exposure, anxiety management, mood management, and guided self-help.
  • Support individuals to attend other appointments, such as social or physical health appointments, which may be impacting their mental health presentation.
  • Integrate with GP practices, work alongside other agencies, and participate in multidisciplinary team discussions.
  • Manage and prioritize a caseload, ensuring timely and effective support to individuals.
  • Support the team in completing physical health checks for patients registered on the Serious Mental Illness (SMI) register.
  • Contribute to service improvement and delivery initiatives.
Requirements
  • GCSE maths and English at C or above, or equivalent adult learning qualification.
  • Level 3 qualification in Care or Level 2 qualification in Care, with a commitment to undertaking the relevant Level 3 qualification on appointment to post.
  • Experience of working with individuals with complex health needs.
  • Desirable: completion of training in psychological intervention and/or counselling skills.
