Job Summary

A dynamic opportunity has arisen for a skilled Mental Health Support Worker to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Mental Health Support Worker, you will work directly with adults who experience various moderate/complex mental health difficulties. You will be expected to deliver psychological interventions as well as provide a 'hands on' approach to meet the person's biopsychosocial needs.

Main Duties

* Work alongside Primary Care Mental Health Practitioners to deliver a plan of care to meet the person's biopsychosocial needs.
* Deliver psychological interventions which may consist of confidence building, graded exposure, anxiety management, mood management, and guided self-help.
* Support patients to attend other appointments such as social/physical which may be negatively impacting upon their mental health presentation.
* Integrate within GP practices, work alongside other agencies, and be involved in MDT/multi-agency discussions.
* Manage and prioritise a caseload.
* Support the team to complete physical health checks for those patients who are registered on the Serious Mental Illness (SMI) register.
* To be involved in service improvement/delivery initiatives.

The Primary Care Mental Health Service (PCMHS) consists of mental health professionals who work into various GP practices across the Sunderland, Washington, and Coalfields locality. The PCMHS deliver care and treatment to adults presenting with moderate/complex mental health difficulties and are an integral part of Primary Care and community working. These roles provide a fantastic opportunity to work across all services who have a shared direction to support the people of Sunderland, Washington, and Coalfields to improve their mental and physical health goals.
