About the Role

Hestia Housing Support is seeking a highly skilled and compassionate Community Engagement Practitioner to join our Mental Health Integrated Voluntary and Community Sector (VCS) Team. As a key member of our team, you will play a vital role in providing non-clinical support to individuals with mental health difficulties, empowering them to achieve independence and improve their quality of life.

Key Responsibilities
  • Provide High-Quality Support: Work closely with adult service users with moderate to severe mental health difficulties, providing individualized support and guidance to help them achieve their goals.
  • Collaborate with Multidisciplinary Teams: Work collaboratively with other professionals and service users to develop and implement recovery plans, ensuring a comprehensive and coordinated approach to care.
  • Assessment and Management: Conduct thorough assessments of client care needs, develop and implement crisis plans, and monitor progress to ensure effective support and care.
  • Outreach and Community Engagement: Engage with local communities, networking with grassroots and statutory services to connect service users with relevant resources and support.
  • Discharge Planning and Support: Support service users in discharge planning from the Community Mental Health Team, building sustainable networks and connections in the community.
  • Psychoeducation and Wellbeing Activities: Facilitate and co-deliver psychoeducation and other wellbeing activities, promoting mental health and wellbeing among service users.
Requirements
  • Understanding of Mental Health Issues: Possess a deep understanding of the complexities and challenges faced by individuals with mental health difficulties.
  • Recovery and Coproduction: Have a strong understanding of recovery principles and coproduction, with experience in implementing these approaches in practice.
  • Autonomy and Teamwork: Demonstrate the ability to work autonomously, as well as collaboratively within a team or group environment.
  • Liaison and Partnership Working: Possess experience in liaising with other agencies and partners, with a strong understanding of the importance of effective partnership working.
  • Treatment Outcomes and Risk Assessment: Have a solid understanding of treatment outcomes, recovery tools, care planning, and risk assessment, with experience in applying these in practice.
  • Mental Health Act and MDT: Possess knowledge and understanding of the Mental Health Act 1983 (amended 2007) and the functions of multidisciplinary teams.
  • Previous Experience: Previous experience in similar roles, such as Support Worker, Mental Health Worker, or Keyworker, would be highly advantageous.
