**Job Summary**

We are seeking a highly skilled and compassionate Care Team Leader to join our team at Hallmark Care Homes. As a key member of our care team, you will be responsible for leading and managing a team of care professionals to deliver high-quality care and service to our residents.

**Key Responsibilities**

* Assist the Registered Nurses, Clinical Care Manager, and Care Manager in delivering care to residents, ensuring continuous assessment, planning, implementation, and evaluation of care plans.

* Supervise, appraise, and perform performance management of the care team, supporting them to achieve their potential and provide excellent care.

* Ensure that each resident receives the meal or diet they have requested and is suitable for their needs.

* Assist with the admission and discharge of residents, including listing of resident's property/clothing.

* Participate in activities provided within the home to promote relationship-centered care.

**Requirements**

* Qualified in a NVQ/QCF Level 3 in Health & Social Care.

* Experienced in a care position for a minimum of 2 years.

* Outstanding communication and interpersonal skills.

* Ability to work effectively in a team environment.
