Job Title: Advanced Practice Nurse

We are seeking an experienced Advanced Practice Nurse to join our team at the NHS. As an Advanced Practice Nurse, you will play a key role in providing high-quality patient care and contributing to the development of our practice.

Job Summary

The successful candidate will be responsible for managing a caseload of patients with complex health needs, providing expert clinical advice, and working collaboratively with other healthcare professionals to deliver effective care.

Key Responsibilities
  • Manage a caseload of patients with complex health needs, providing expert clinical advice and care.
  • Work collaboratively with other healthcare professionals to deliver effective care, including GPs, practice nurses, and other specialists.
  • Develop and implement care plans, taking into account the individual needs and circumstances of each patient.
  • Provide education and support to patients, carers, and other healthcare professionals to promote healthy lifestyles and disease prevention.
  • Contribute to the development of our practice, including the development of policies, procedures, and guidelines.
Person Specification
  • Registered General Nurse with a minimum of 2 years' post-registration experience.
  • Non-Medical Independent Prescriber qualification.
  • Experience of working in a primary care setting, preferably in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
