Position Overview
An excellent opportunity has arisen for a Band 3 Patient Flow Coordinator.
The Patient Flow Team is dedicated to ensuring that patients are placed in the appropriate care settings at the right time.
This team plays a crucial role in supporting the Emergency Department by optimizing patient movement throughout the healthcare facility.
The Patient Flow Coordinator will collaborate with Site practitioners to manage efficient patient transitions by identifying available capacity within ward areas and facilitating prompt discharges.

Key Responsibilities
- Maintain an up-to-date and precise bed status, including information on planned discharges, admissions, GP referrals, and additional capacity.
- Collaborate with wards, the Emergency Department, Integrated Discharge Team, and multi-disciplinary teams to ensure timely patient transfers.
- Communicate effectively with clinical and non-clinical support services to address issues impacting patient flow.
- Update and maintain databases to ensure accurate reporting on bed utilization and other necessary data for daily and weekly managerial reports.
- Work closely with the Emergency Department to develop solutions that minimize potential delays, escalating issues to capacity managers and Care Group managers as necessary.
- Liaise with staff in the Emergency Department and assessment units to ensure smooth operations and patient flow.
- Keep the nurse in charge of the Emergency Department and Assessment Medical Unit informed about bed availability.
- Ensure timely transfers of patients from the Emergency Department or between wards, escalating any delays to relevant managers.
- Coordinate ambulance bookings in accordance with Department of Health guidelines and monitor ambulance discharges.
- Engage with alternative patient transport services as required.
- Support the implementation of the trust's escalation policy.
- Communicate effectively to address potential complaints and concerns.
- Collaborate with GPs and other Allied Health Professionals to refer patients to appropriate teams.
- Work with Clinical Commissioning Groups and Social Care Partners as necessary.
- Assist in the completion of the weekly operational plan and compile information for daily bed meetings.
