We are seeking an experienced Registered General Nurse to join our Care Community Teams as a Bank Nurse. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality nursing services to patients in their own homes.
Key Responsibilities- Deliver nursing services and holistic assessments of patients' health needs using a person-centred approach.
- Work alongside various professionals to provide quality and seamless care to patients.
- Coordinate patients' care needs, avoiding unnecessary admission to hospital or long-term care settings.
- Develop and maintain effective relationships with patients, families, and healthcare teams.
- Contribute to the development and implementation of care plans, ensuring patient-centred and evidence-based practice.
