Community Health Nurse

We are seeking a highly skilled and motivated Community Nurse to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality nursing services to patients in their own homes, promoting independence and improving health outcomes.
Key Responsibilities- Deliver holistic assessments and care plans to patients, ensuring a person-centred approach.
- Work collaboratively with multidisciplinary teams to provide seamless care to patients.
- Coordinate patient care needs, avoiding unnecessary hospital admissions or long-term care settings.
- Develop and maintain effective relationships with patients, families, and carers.
- Contribute to the development and implementation of community nursing services, ensuring they meet the needs of patients and the community.

We are looking for a qualified Registered General Nurse with recent training and experience in community nursing. You will have excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team. A degree in Professional Practice or Student Mentorship would be desirable.
Please note that applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. New entrants to the NHS will commence on the minimum of the scale stated above.
