Community Health Nurse

We are seeking a highly skilled and motivated Community Nurse to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality nursing services to patients in their own homes, promoting independence and improving health outcomes.
Key Responsibilities- Deliver holistic assessments and care plans to patients, ensuring a person-centred approach.
- Work collaboratively with multidisciplinary teams to provide seamless care to patients.
- Coordinate patient care needs, avoiding unnecessary hospital admissions or long-term care settings.
- Develop and maintain effective relationships with patients, families, and carers.
- Contribute to the development and implementation of community nursing services, ensuring they meet the needs of patients and the community.

We are looking for a Community Nurse who:
- Is a Registered General Nurse with evidence of recent training.
- Has experience of working in community nursing and multi-disciplinary teams.
- Has a strong understanding of the Public Health Agenda and local and national priorities.
- Can demonstrate high standards of nursing practice and a commitment to ongoing learning and development.
