About the Role

We are seeking a skilled and compassionate Healthcare Assistant Practitioner to join our team at Nottingham CityCare Partnership CIC. As a key member of our Clinical Beds team, you will play a vital role in providing high-quality, patient-centered care to individuals in our community settings.

Key Responsibilities
  • Work collaboratively with Nurses, Physiotherapists, Occupational Therapists, and Rehabilitation Support Workers to assess and treat patients in our Clinical Beds setting.
  • Support the work of clinicians, providing assessments of needs and working across professional boundaries and within legislation, policies, and procedures.
  • Develop and implement individual care plans, utilizing practical and theoretical knowledge under the supervision of a registered practitioner.
  • Conduct daily reviews of patients' health and social care needs, taking appropriate action to achieve planned aims.
  • Perform area-specific clinical competencies to enhance the delivery of patient care, as delegated by the registered practitioner.
  • Participate in the development and implementation of rehabilitation programs, as agreed with a registered professional.
Requirements
  • Knowledge of professional area procedures and practices (nursing, physiotherapy, or occupational therapy).
  • Knowledge of Health and Social Care provision.
  • Understanding of the concepts of assessment and care planning.
  • Awareness of Rehabilitation and/or Nursing Care processes.
  • Effective communication skills (verbal and written) and good interpersonal skills.
  • Ability to prioritize and plan workload.
  • Ability to follow the Health Care Support Workers code of conduct policy and maintain professional boundaries.
Desirable Qualifications and Experience
  • Experience of multi-disciplinary team working in a Health Care, Social Care, or Voluntary Care setting.
  • Experience of working with individuals and/or groups within a Health Care, Social Care, or Voluntary Care setting.
  • Experience of working with individuals who have long-term conditions.
  • Experience of record keeping and accurate data recording.
